Evaluation of cataract progression after pars plana vitrectomy in young patients with rhegmatogenous retinal detachment

ABSTRACT

Objective:

To evaluate the rate and risk factors associated with cataract progression post pars plana vitrectomy in young patients with rhegmatogenous retinal detachment.

Method:

This retrospective study was conducted between January 2020 to January 2025 involving patients with ages between 18 to 45 years old undergoing pars plana vitrectomy for uncomplicated rhegmatogenous retinal detachment.

Results:

A total of 125 eyes (from 125 patients) were recruited. Cataracts developed in 105/125 (84%) eyes after pars plana vitrectomy. Nuclear sclerotic cataract developed in 55/105 (52%), while posterior subcapsular cataract in 42/105 (40%), and cortical cataract in 8/105 (8%) eyes respectively. Cataracts developed more frequently in macula-off rhegmatogenous retinal detachment (76%) compared to macula-on rhegmatogenous retinal detachment (24%) (p = 0.014). Longer surgical time of more than 60 minutes was associated with higher rate of cataract formation in 60% of patients.

Conclusion:

Cataract formation after pars plana vitrectomy is a known complication. It can happen in young patients and cause visual significant disturbances as well. Even though the rate of formation is lesser in patients younger than 35 years old, the condition needs to be addressed as they are young functioning adults.
