This paper describes the intraoperative refraction for intraocular lens (IOL) power calculation. Report a case of a 67 year-old male patient reporting a progressive loss of visual acuity in the left eye (OS). Examination revealed best corrected visual acuity (BCVA) of 20/60 (-21,50 -4,00 X 170) related to adherence of silicone oil to the IOL optic. The intraocular lenses have been used in visual rehabilitation after cataract surgery since 1959 with a large success. However, biomaterial changes in the optic zone of these lenses have been described in the last years reducing visual acuity after surgery. This case reveals a significant snowflake opacification of the optic zone of polymethylmethacrylate intraocular lens followed by a discussion about this event and the indication for IOL exchange.
