This 30-year-old patient had a high myopia and with-the-rule astigmatism. Her best-corrected-visual-acuity was 20/20 with -6.75 -2.50 x 10. Her cornea had an asymmetric bowtie, and we were concerned with laser vision correction. We then decided to implant a toric phakic intraocular lens (Visian ICL® toric) and, 1 week after surgery, she was 20/20 J1 uncorrected. Note the central anterior vaulting of the phakic lens away from the crystalline lens, as well as the central hole to allow for aqueous humor to flow through it.
