Fuchs’ heterochromic iridocyclits or Fuchs’ Syndrome is a kind of uveitides relatively uncommon. Affect equally both genders, between 20-45 years old, in the classic features include a chronic unilateral nongranulomatous inflammation in anterior uvea, insidious onset, low-grade in activity, and unresponsive to corticosteroid therapy. Usually has a good prognosis, except for the development of cataract and glaucoma, pathologies associated with syndrome. This paper describes the intraoperative refraction for intraocular lens (IOL) power calculation. Report a case of a 67 year-old male patient reporting a progressive loss of visual acuity in the left eye (OS). The intraocular lenses have been used in visual rehabilitation after cataract surgery since 1959 with a large success. However, biomaterial changes in the optic zone of these lenses have been described in the last years reducing visual acuity after surgery. This case reveals a significant snowflake opacification of the optic zone of polymethylmethacrylate intraocular lens followed by a discussion about this event and the indication for IOL exchange.
