Surgical approaches to lateral brow lift

ABSTRACT
Eyebrow position plays a central role in facial harmony and emotional expression. Lateral brow ptosis is a common condition associated with aging, tissue laxity, and individual anatomical characteristics, with aesthetic and, in more severe cases, functional consequences. The main treatment techniques include direct brow lift (Castanhares technique), transpalpebral approach, internal browpexy, subcutaneous temporal lift, brassiere suture, and endoscopic lift. Each technique carries specific indications based on the degree of ptosis, the patient’s anatomical profile, and aesthetic expectations. Literature highlights the need for individualized technique selection and underscores the absence of a universally superior method. The variety of available approaches reflects the anatomical complexity of the periorbital region and the need for individualized surgical planning. Choosing the optimal technique should consider clinical and aesthetic factors as well as the surgeon’s experience.
