Preexisting corneal astigmatism, present at the time of cataract surgery, is reviewed in detail throughout this article on its most important aspects such as occurrence rates, clinical relevance and current treatment options. Special emphasis is given to the latter aspect. Each method’s rationale, advantage and limitation ishigh lightened. Comparisons between treatment options, whenever possible, are also provided.

Five cases of intraocular lens (IOL) opacification in patients implanted with the Mediphacos Ioflex® IOL are described. Clinical data in each case was obtained from the patient’s medical record. The five explanted IOLs underwent gross and light microscopic analysis. Selected lenses were processed for further evaluation and multiple sagittal cuts were stained with the alizarin red and von Kossa methods. This is a case report of a 45-year-old anisometropic woman with stage 2 keratoconus submitted to refractive lens exchange employing SN60T5 AcrySof Toric intraocluar lens on her left eye. Phacoemulsification followed by proper lens implantation was accomplished successfully. On postoperative period, favorable refractive and visual outcomes were obtained. The use of toric intraocular lenses may be an option in the refractive treatment of stage 2 keratoconus in selected cases.

The intraocular lenses have been used in visual rehabilitation after cataract surgery since 1959 with a large success. However, biomaterial changes in the optic zone of these lenses have been described in the last years reducing visual acuity after surgery. This case reveals a significant snowflake opacification of the optic zone of polymethylmethacrylate intraocular lens followed by a discussion about this event and the indication for IOL exchange.
