ABSTRACT The objective was to report a case of severe eyelid ptosis associated with Kearns-Sayre Syndrome (KSS), with 12 years of postoperative follow-up. A patient diagnosed with KSS presented with progressive eyelid ptosis since childhood and underwent bilateral frontalis suspension using silicone rods. Clinical follow-up included evaluation of eyelid symmetry, ocular motility, and visual acuity.
