The detachment of Descemet’s membrane can be a serious complication following cataract surgery, leading to severe corneal edema and reduced visual acuity. This report describes an unusual case of Descemet’s membrane detachment 6 months after successful phacoemulsification, documented by anterior segment optic coherence tomography (OCT; RTVue, Optovue). The intraocular lenses have been used in visual rehabilitation after cataract surgery since 1959 with a large success. However, biomaterial changes in the optic zone of these lenses have been described in the last years reducing visual acuity after surgery. This case reveals a significant snowflake opacification of the optic zone of polymethylmethacrylate intraocular lens followed by a discussion about this event and the indication for IOL exchange.
