Real-world comparative longitudinal outcomes of iStent inject® versus iStent inject® W for open-angle glaucoma

ABSTRACT

Objective:

To compare the efficacy and safety of the iStent inject® and iStent inject® W in patients with primary open-angle glaucoma (POAG) undergoing standalone or combined implantation with phacoemulsification.

Methods:

Retrospective longitudinal study including 103 eyes with mild-to-moderate POAG. Intraocular pressure (IOP), number of medications, absolute and relative success rates, survival analysis, and safety outcomes were evaluated over 12 months.

Results:

Fifty-nine eyes received iStent inject® (Group 1) and 44 received iStent inject® W (Group 2). Mean IOP decreased significantly in both groups: Group 1: 15.7 mmHg to 13.8 mmHg (p<0.001) and Group 2: 16.4 mmHg to 13.5 mmHg (p<0.001), with no statistically significant difference between the groups in final IOP (p=0.464). The mean reduction in the number of medications was 80%, with no differences between the groups (p=0.851). Absolute success (IOP < 18 mmHg without medication) was 83.1% and 84.1% in groups 1 and 2, respectively (p=0.554). There were no significant differences between the devices in any of the outcomes evaluated. Both showed an excellent safety profile.

Conclusion:

The iStent inject® and iStent inject® W showed similar efficacy and safety in the surgical management of mild-to-moderate POAG, with no clinically significant differences at 12 months.
